Preventive PLUS is your U.S. Citizenship and Immigration Services (USCIS) designated civil surgeon and is authorized to perform USCIS medical examinations.

When you apply for a green card (adjustment of status) in the United States, you need to have a medical examination. The exam must be done by a doctor who is authorized by U.S. Citizenship and Immigration Services (USCIS). USCIS designates certain doctors (also known as civil surgeons) to perform the medical exam required for Green Card applicants.

Medical examinations in the United States are typically required for adjustment of status cases (Form I-485), and may also be required for requests for “V” nonimmigrant status (Form I-539).
Print out and bring the most recent version of Form I-693, Report of Medical Examination and Vaccination Record, to your medical exam appointment: Fill out Part 1 of Form I-693 but do not sign until the civil surgeon instructs you to do so. The civil surgeon will use Form I-693 to document the results of your medical exam.
The designated civil surgeon will complete, sign and seal Form I-693 and any supporting documents in an envelope. You must submit the sealed envelope to USCIS as directed in the Form I-693 instructions.

Such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.
Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease)
Please bring your medical insurance card
Payment (check with the Preventive PLUS about acceptable forms of payment).
Immigration medical examination faqs
In May and June 2012, USCIS hosted a series of seminars for civil surgeons to provide information about the immigration medical exam and answer questions.
During the seminars, USCIS subject matter experts provided an overview of Form I-693, Report of Medical Exam and Vaccination Record and the immigration medical exam and shared information about the required assessments for communicable diseases of public health significance; vaccination requirements; mental and physical disorders with associated harmful behavior; and drug abuse and addiction.
